We bill for all deductibles and co-insurance as documented on the EOB you will receive from your insurance plan.

It depends on the test, see below for the average TAT:

  • COVID-19 – 24-48 hours
  • Toxicology - screen results are available within 12 hours of sample receipt. Confirmation results are available 48 hours after sample receipt.
  • Chemistry Blood Tests – 24 hours in most cases

Please complete a supply request form and the requested supplies will be delivered to your facility in 24-48 hours.

Once your doctor has determined that you are a good candidate for PGx testing, a preauthorization will need to be completed. Your doctor will complete a patient fact sheet that includes your demographic information, medication history, and the desired outcome of the testing. Once these forms are completed the laboratory will work with your insurance to determine if the testing will be covered and will communicate with you about any out-of-pocket cost prior to collecting and processing your sample.
